A 40-year-old man had PRK in both eyes with a broad-beam laser. Initially, UCVA was a very good, but it has decreased in the past 3 years. Topography shows a slightly decentered ablation zone. Although the patient does not report glare or halos, he could not detect any number on the Nyktometer with an additional bright light source. The patient requests correction of the residual myopia in his right eye. What would you advise?

A videokeratography-derived system – the KISA% – incorporates 4 quantifiable phenotypic features of keratoconus into 1 numerical expression. The magnitude of this number correlates well with the severity of clinical keratoconus and can be used to accurately separate eyes with keratoconus from normal eyes.
