The combination of small pupil hard cataract presents unique surgical challenges. small pupil can arise from various causes such as advanced age, chronic ocular conditions (e.g. uveitis),systemic diseases e.g. diabetes. the limited pupillary size hinders adequate visualization of lens, complicating the capsulorrhexis. here mutiple ways shown for phaco using iris hooks, rings and pupilloplasty.
