Primary lens exchange was performed for a broken IOL seen at the end of a Phaco-Hydrus surgery. One optic-haptic junction was partially broken. The IOL was floated and maneuvered out of the bag with viscoelastic. A twist maneuver was tried with McPherson forceps to remove the entire IOL, but one haptic auto amputated. The other broken haptic followed by the optic was removed and new IOL inserted.
