Syed S. Hasnain MD believes that the orderly loss of nerve fibers is the only true lead we have in solving the mystery of primary open-angle glaucoma. While considering elevated IOP as the definitive cause of glaucoma, and the orderly loss of nerve fibers as a salient feature, he proposes that glaucoma may be a two-stage disease. Glaucoma may not be an optic neuropathy, but an optic disc axotomy.
