This study describes an “open-sky” implantation with penetrating keratoplasty for scleral fixation of a Reper artificial iris IOL in a complete traumatic corneal pannus, aphakia and aniridia. Three sclerotomies were made for fixation, and a donor cornea was sutured. The IOL was secured with 10-0 Prolene scleral sutures.
