One eye, six years, three surgical procedures. This case presents the challenging management of a 45-year-old with Macular Dystrophy. After a DALK and subsequent phacoemulsification with a toric IOL, she developed endothelial decompensation. The choice of DSAEK addressed this rare complication while preserving the IOL's astigmatic correction, leading to an excellent and stable visual outcome.
