A 52-year-old man with corneal decompensation caused by an inverted multipiece IOL, with a haptic touching the endothelium, underwent an in situ IOL flip with simultaneous scleral fixation without explantation. Although IOL positioning was successfully restored, corneal decompensation persisted postoperatively, and DSAEK may be needed for visual rehabilitation
