This patient had traumatic cataract surgery elsewhere with subluxated IOL, corneal and macular oedema, mydriasis, and uncontrolled intraocular pressure. We performed an Ahmed valve, vitrectomy, fragment retrieval, corticosteroid intravitreal injection, IOL repositioning with iris suture, and personalized iris cerclage avoiding the IOL suture. The patient recovered with good vision and IOP.
