During my first solo cataract, a routine case without risk factors unexpectedly evolved into a surgical challenge. After uneventful phacoemulsification and IOL implantation, sudden inferonasal zonular detachment with vitreous prolapse occurred. Triamcinolone-assisted bimanual anterior vitrectomy successfully removed vitreous, preserved IOL stability, and achieved 0 LogMAR vision at 1 month.
