We present a case of advanced keratoconus undergoing DALK. During partial trephination, inadvertent anterior chamber entry occurred over 4 clock hours. Rather than converting to PK, the area was sutured and a type 1 Big Bubble achieved with a 26G needle. The procedure was completed as DALK, highlighting that lamellar keratoplasty can be preserved despite intraoperative complications.
