This report describes complete dislocation of the IOL–bag–CTR complex into the vitreous cavity. The complex was relocated into the anterior segment after face-down positioning and temporarily fixed to the cornea. Definitive scleral fixation was performed in the operating room, resulting in stable centration and satisfactory visual recovery.
