This study evaluated ICL sizing prediction using AI-based analysis of 3D corneal diameter profiles from Scheimpflug imaging. Among 350 eyes, delta CD strongly correlated with vault prediction error (r=0.9). Beyond 1.2mm delta CD, error rose by 127µm per 0.1mm. The AI decision tree achieved an AUC of 0.9, suggesting 3D CD metrics can improve vault predictability and optimize ICL sizing.
