This video demonstrates two approaches to complex anterior segment reconstruction with an artificial iris-IOL (AI-IOL) complex fixated by intrascleral fixation. Part 1: open-sky “triple” with PKP and AI-IOL fixation. Part 2: IOL exchange and vitrectomy with AI-IOL fixation in a closed chamber. These cases highlight technique selection and intraoperative decision-making.
