A 50 year old presented with a subluxated 3 piece IOL. The surgeon elected to repositiom the IOL using scleral fixation technique. Although intraoperative repositioning was successful, the IOL flipped the following day, leaving no option but explantation. This case illustrates both a safe technique for repositioning a scleral fixated IOL and a practical method of explantation.
