During irrigation/aspiration, hydrostatic pressure from the I/A handpiece pushes the posterior capsule backwards concavely allowing easy aspiration of lens cortex. Occasionally, the posterior capsule is extremely floppy and may follow the lens cortex into the aspiration port during suction. This movie illustrates this and discusses the reasons and steps to take to avoid posterior capsule rupture.
