This video shows a 78-year-old male with traumatic mydriasis and cataract with zonular weakness from a pickleball injury. Zonular instability is seen during cortex removal. A CTR and toric IOL are placed, followed by pupillary cerclage. The surgeon shifts from a temporal to superior position for inferior iris pupilloplasty.
