An Argentinian flag anterior capsule tear occurred during capsulorrhexis in a moderate nuclear sclerotic cataract case. We demonstrate safe lens removal with hydrodelamination, divide and conquer technique, and bimanual irrigation and aspiration, with successful in-the-bag one-piece acrylic intraocular lens placement, and no posterior extension.
