A 40-year-old male with bilateral penetrating keratoplasty sustained blunt trauma to the left eye, treated with vitrectomy and silicone oil. The graft became opaque, and the patient remained aphakic. This video demonstrates a combined optical penetrating keratoplasty, silicone oil removal, and scleral fixation of an intraocular lens.
