This case describes a 76-year-old female with severe acanthamoeba keratitis refractory to maximal medical therapy and a therapeutic penetrating keratoplasty. Despite intervention, the patient developed progressive corneal necrosis, scleral extension, and frank perforation. Definitive eradication of infection and restoration of tectonic stability were ultimately achieved with sclerokeratoplasty.
