The SIRF technique uses modified iris hooks, riveted transconjunctivally, to scleral-fixate dislocated IOLs at the anterior capsulorhexis. It requires only small incisions, minimizes IOL manipulation, allows adjustable centration, and is effective even in small pupils. It is applicable to many IOL types—including toric, multifocal, accommodating, and PMMA—if the anterior rhexis is intact.
