This film shows how intraoperative optical coherence tomography (OCT) extends beyond conventional visualization and tactile feedback in deep lamellar keratoplasty. Real-time imaging provides precise depth guidance, detects microperforations, and confirms graft–host apposition. Pearls and pitfalls highlight how OCT enhances safety and makes this demanding procedure more reproducible for trainees.
