A 67-year-old female with ICE syndrome and corneal decompensation underwent Endo-in DMEK, achieving clear graft and visual recovery to her baseline potential which is 20/40 with a hard contact lens. This technique minimizes intraocular manipulation and improves outcomes, particularly in complex cases with a shallow anterior chamber and peripheral anterior synechiae, as seen in ICE syndrome.
