During emulsification of grade 4 hard cataract the last piece emulsification poses a challenge. It needs high power and vacuum settings to manage. Surge at this point leads to posterior capsule touch and rupture. An IOL inserted between the quadrant and posterior capsule saves the mishap. The advantage is it does not need removal and is left in the bag.
