Marfan syndrome can present numerous challenges during cataract surgery. Diffuse zonulopathy may limit counter-traction during capsulorhexis, leading to capsular tear-out and difficulties in subsequent surgical stages. This film demonstrates innovative use of suction cup and pulse capsulotomy to create a stable anterior capsular opening, allowing for safe phacoemulsification and case completion.
