Lens dislocation is the main ophthalmic complication of Marfan syndrome and is caused by rupture of the zonula of Zinn (Z). During intraocular lens suture surgery in patients with Marfan syndrome, we observed the ciliary sulcus using an intraocular endoscope and found hypoplasia of the ciliary process. Rupture of Z may be caused by hypoplasia of Z resulting from hypoplasia of the ciliary process.
