We present a case of an 81-year-old male with severe miosis and POAG undergoing cataract surgery with a toric lens and MIGS with OMNI. This video highlights five key strategies to improve patient comfort and surgical efficiency: using shugarcaine before Provisc, thorough Provisc removal, Malyugin ring removal prior to MIGS, use of a Viscoat plug to limit Provisc extrusion, and incision hydration.
